FREQUENTLY ASKED QUESTIONS

Do I need to bring anything to the exam?


  • You will need a valid State ID or a driver's license. 
  • Bring any medications you are taking with you or a list from your pharmacy or doctor.
  • Bring your prescription eyeglasses if you wear them.


What if I cannot make the appointment?


  • You will need to call your adjudicator (case worker) in Springfield to let them know.  The number is 1-800-225-3607. The adjudicator's name and extension can be found on the letter you received from the State. Please call our office as well to let us know at 312-940-8839.  We cannot set up another appointment, but must receive approval from the State to do so.

How long do exams take?It will depend on the complexity of your case and if you are seeing more than one physician.  Please check all paperwork to make sure you have correct dates and times for the exams.  If you are having more than one exam you will receive two letters from the State. The exams will be on the same day, but two different times.

Do we get reimbursed for transportation?


  • You do not get reimbursed by our office.  We are an independent facility and not part of the State.  We are an approved site for evaluations only.

What if I don't speak English?


  • We do have Spanish-speaking staff, but we suggest that you bring a family member or friend that speaks English.  If you are having a psychological or psychiatric  evaluation, an interpreter will be set up for you by the State of Illinois.

Food and beverages (other than water), are prohibited in the office.


There is no cell phone usage allowed in the waiting area.  You must exit to the hallway to use your phone.


We ask that you only bring one person to assist you if you need help and not multiple people.  Please find child care so that children do not cause disruption in the waiting area.  There are somtimes very ill people who are waiting to be seen.  Thank you for your cooperation.
