During Covid our office has instituted additional precautions to keep everyone safe.

Guidelines:

Do not travel outside of Illinois for at least 2 weeks before your office appointment.  

If you have had Covid 19 you must have a a Covid test and send that to the office prior to you coming in.

If you are experiencing any symptoms of Covid 19 such as cough, fever, headache, shortness of breath, diarrhea or vomiting, you will not be seen until you are tested and have a negative test.

You must wear a mask that covers your nose and your mouth when entering the office.  If you have a condition that precludes you from wearing a mask, you must have a face shield.

Your temperature will be taken upon entering the office.

It is necessary that upon arrival to our building you call the office 312-940-8839 to get permission to come up to the suite. This is done to reduce the exposure to other persons.

Only 1 person can assist the claimant-if that is required-up to the suite and then that person must wait outside until the claimant has been seen.
